Guest Win/Loss Request

A Winners Club card is the first step in establishing the kind of personal relationship that you would expect from Quil Ceda Creek Casino. With a Winners Club card you become one of our most valued guests. Guests that are Winners Club members are eligible to receive a win/loss statement by mail. Please click on the link at the bottom of this section and print out the attached document.

1. Please complete all sections of the form:
   1. Year ending, (the year you want your statement to cover).
   2. Print your full name.
   3. Birth date mm/dd/yyyy.
   4. Your Winners Club number, if known.
   5. Your complete address.
   6. Check how you would like to receive your Win/Loss letter.
   7. Your signature.
2. Enclose a copy of your current state identification card or drivers license. We will not be able to process your request without this.
